INFORMED – ENGAGED – EMPOWERED Please join us May 16th, 2009 for a one-day, eight hour conference in San Francisco, California for patients and health care providers to come together to build the necessary skills and share innovative solutions to succeed in today's complex health care system. We will provide patients the information, strategies and skills they need to become responsible members of their health care team, to work in true partnerships with their providers and to greatly reduce their chances of encountering adverse events and medical errors. We also plan to include a forum for providers to share their best practices on how to build successful provider/patient teams. The event features expert speakers in the following areas: The collaborative learning sessions will include the experiences and the expertise of patients, providers and professional advocates. Our goal is to maximize the positive impact the public can have on health care quality and to provide patients with the tools to build winning and lasting partnerships with their providers. We believe that those who come to see themselves as capable and confident members of their medical team will gladly accept their role as stewards of health care that is safe, effective and compassionate Our goal is to provide attendees with a blueprint of how to become an empowered patient and an active participant in their health care team. Participants will be able to implement immediate changes that will have an immediate and a long-term impact on the quality of their medical care. Most important, attendees will be able to share their new health knowledge with family and friends who may also be seeking health care relationships that are empowered and patient-centered. This event is appropriate for all members of the public, medical patients, health care practitioners, anyone living with chronic health conditions, caregivers, health care advocates and those individuals interested in making empowered health care a reality.
